Does Insurance Cover Doula Services in Maryland? Your Complete Guide

Shannon MacAdams is a certified birth doula serving Annapolis and Anne Arundel County, MD. Doula services in Maryland may be covered by Medicaid, Blue Cross Blue Shield, and other insurers — and Shannon can help you figure out your benefits before your first visit. TRICARE coverage is available separately through the CBSD program for participating providers; see the section below.

One of the most common questions I hear from families in Annapolis is: “We want doula support, but can we afford it?” The answer has changed significantly in the past few years. Maryland has become one of the most doula-friendly states in the country when it comes to insurance coverage — and starting January 1, 2026, even more families have coverage than before.

Here’s a plain-English guide to insurance coverage for doula services in Maryland, and how to find out if you’re covered.

Maryland Medicaid: Full Doula Coverage Since 2022

Maryland Medicaid began reimbursing doula services on January 1, 2022. If you have Maryland Medicaid (also called HealthChoice), you are eligible for:

  • Up to 8 prenatal or postpartum doula visits — each up to 60 minutes, in person or by telehealth
  • Continuous support during labor and delivery — in person at the hospital or birth center
  • No cost to you — covered under your Medicaid benefits

To qualify, you must be a Maryland Medicaid member who is currently pregnant, or who has been pregnant within the last 180 days. Coverage is broad — it’s designed to reduce Maryland’s maternal mortality disparities, particularly for Black and brown birthing families.

To access your Medicaid doula benefit, contact your Managed Care Organization (MCO) — the company that manages your Medicaid plan. Ask: “Can I get doula services covered under my benefits?” They will connect you with approved providers in your area. You can also reach the Maryland Department of Health’s Medicaid Doula Program directly at health.maryland.gov.

Private Insurance: Some Plans Cover Doulas — Here’s How to Check

Private insurance coverage for doulas varies widely. Some employer-sponsored plans and marketplace plans include doula services as a maternity benefit — the fastest way to find out is to call the member services number on your card and ask directly. BCBS, Aetna, Cigna, and other insurers handle this differently plan by plan, so don’t assume you’re not covered until you ask.

Important 2026 update: CareFirst BlueCross BlueShield added doulas as a covered maternity benefit starting January 1, 2026. This applies to CareFirst BlueChoice, Blue Value Plus, and BlueChoice Advantage HDHP plans — which includes many federal employees through the Federal Employees Health Benefits (FEHB) program and the Postal Service Health Benefits (PSHB) program. If you work for the federal government or USPS and have CareFirst, your doula may be covered starting this year.

Coverage details vary by specific BCBS plan. Call the member services number on the back of your card and ask: “Are doula services covered under my maternity benefits for 2026?” CareFirst FEHB members can call (888) 789-9065; PSHB members can call (833) 489-1316.

Shannon can help you navigate this during your free consultation. She’s worked with families in Annapolis who have BCBS through their employers as well as ACA marketplace plans.

TRICARE: Coverage for Military Families

TRICARE covers doula services for eligible military families through the Childbirth and Breastfeeding Support Demonstration (CBSD). Doula care is reimbursable for TRICARE Prime, Prime Remote, and Select beneficiaries when provided by a doula enrolled in the CBSD program. This is relevant for families near Fort Meade, the United States Naval Academy, and Joint Base Andrews.

Please note: Shannon’s Doula Care is not currently enrolled in the CBSD program, so TRICARE cannot be billed directly for her services. If you’d like to use TRICARE coverage for doula care, you’ll need to find a CBSD-participating doula through your TRICARE plan administrator.

If you’d like to work with Shannon, military families have several other ways to make doula care affordable — flexible payment plans, FSA/HSA funds, employer benefits like Carrot Fertility, and CareFirst BCBS (which added doula coverage in 2026 for civilian-employed spouses). Reach out during a free consultation to discuss what works for your family.

Other Insurance Plans: FSA, HSA, and More

Even if your specific health insurance plan doesn’t cover doula services directly, you may be able to use:

  • Flexible Spending Account (FSA) — Doula services are an eligible FSA expense under IRS guidelines
  • Health Savings Account (HSA) — Similarly eligible for HSA reimbursement
  • Carrot Fertility — Covers doula services for employees at companies that offer Carrot benefits
  • Progyny — Some employers offer Progyny, which includes doula coverage

Shannon also offers flexible payment plans so that no family in Annapolis or Anne Arundel County has to go without birth support because of cost. Reach out during your free consultation to discuss what works for your situation.

How to Check If Your Insurance Covers a Doula

The fastest way to find out is to call the member services number on the back of your insurance card. Here’s exactly what to say:

“Does my plan cover doula services during pregnancy and birth? What is the billing code I should provide to my doula? Do I need prior authorization?”

Common billing codes for doula services include CPT 99499 (unlisted evaluation and management service) and doula-specific Medicaid codes. Shannon can provide the billing information you need when you connect with your insurer.
